1. Click on the Configuration tab and the Documents Library will appear.
2. Select 'Link Forms to Activities' from the View drop down list.

3. Click on Arrow.jpg to _{Image Added} to show all unassigned activities.

4. Select one activity and drag to the grey boxed area which corresponds to the observation form.

5. Once unassigned activities have been linked to a form, click .
